Concussions
In accordance with the Louisiana Youth Concussion Act, we require parents and athletes to read and sign our
Concussion Statement for Parents and Athletes when registering for any activity.
* Both parents and athletes are required to sign a new form for every activity.
For athletes 12 years old and under, only parents are required to sign the forms.
No athlete may participate without a signed form on file.
